Integrated Urgent Care

Our clinical assessment service provides clinical support to patients 24 hours a day, 365 days a year. Our experienced multidisciplinary primary care teams receive direct referrals from NHS 111 and provide remote and face-to-face consultations, as well as home visiting services. As part of this, we also assess patients referred to ED by 111 and, when suitable, offer alternative care based on clinical need.

We have two Integrated Primary Care Centres for face-to-face consultations, located at Hook Surgery and Farnborough Centre for Health. Our fully equipped home visiting vehicle fleet is based at these locations.

Urgent Care

Aldershot Urgent Care Centre (AUCC)

We work in partnership with Salus Medical Services (GP Federation in North East Hampshire and Farnham), to operate an Urgent Care Centre based at Aldershot Centre for Health.

This service provides urgent care for residents of Aldersot, Farnborough, Fleet, Yateley, Farnham, and Surrey Heath. The service is accessed via 111, your local GP (Aldershot only) and via re-direction from Frimley Park A&E department. The service also offers walk-in appointments for children under the age of

Emergency Department Streaming

At our ED Streaming service at Frimley Park Hospital, patients are assessed on arrival by an emergency department clinician, and if suitable for primary care redirected to our experienced team to provide the right care for the right patients. This also helps to reduce waiting times for treatment and pressure on emergency department.

VirtualCare@Home

Our award-wining Virtual Care at Home service operates seven days a week to remotely monitor patients over the age of eighteen with the following respiratory conditions:

  • COVID-19
  • Asthma
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Acute Respiratory Infection

The service is in place to help you remain at home while you recover and identify early if you need any further clinical support.

Patients can be referred via their own GP practice, NHS 111, Hampshire Hospitals NHS Foundation Trust’s hospital and Urgent Community Response Team.

GP Enhanced Access

We work closely with local GP practices to provide clinics for routine, pre-booked appointments at weekends that may be more convenient for you. We offer a face-to-face clinic which is based at Farnborough Centre for Health, as well as several remote consultation clinics. We have full access to your electronic patient record to provide you with a safer and more effective service. These services are accessed via your GP Practice.

Community Services

We recognise that providing care in our communities can make a real difference for local people. Our teams provide a seasonal flu response for local residential and nursing homes, as well as medical cover for a number of local community hospitals and hospices out-of-hours.
